KLA Presidential Award Nomination Form

    Thank you for nominating a colleague for a KLA Presidential Award!

    It is the goal of the KLA Presidential Awards Committee to honor exemplary service by members of the Kansas Library community. Nominees for the KLA Presidential Awards must meet specific criteria to be selected for their award. The criteria is posted on the KLA webpage. Please read through the criteria carefully before making your nomination.

    If you would like to make a nomination, please fill out this form in its entirety.

    Nominators must submit a completed application, which includes all relevant fields filled out on this form. If you would like to include additional supporting letters of recommendation, they can be uploaded with this application or can be sent via email to (Word or PDF) to: gloria.creeddikeogu@ottawa.edu. This application can be considered as one of the supporting letters.

    The deadline for the receipt of the completed application is September 14, 2018.

    The Awards will be presented in an awards ceremony at the KLA Annual Conference at the Wichita Hyatt, October 23-26, 2018.

      Library Advocate (Nominees not employed in library service either now or in the past)
      New Professional (Nominees who have earned a professional degree within the last five years, and who are currently employed in library service)
      Library Personnel (Nominees who are currently employed in library service or have retired from such service)
      Meritorious Service (Nominees who have provided exemplary, sustained service)
